Please download the forms below and email them to us at [email protected] or fill them out and bring them with you to your first appointment.

How to fill out the forms:

Click the form you need to download them, fill the blanks within the word document, click "File" and then "Save as" in the top left corner of the Word App. Save the file as your first name and last initial (for example, "Emily H"), and then attach the file to an email and send to [email protected] prior to your first appointment.
